Traditional Funeral, Cremation Survey (CS450 1)

This survey is for funeral directors who wish to inform the community that they provideSurvey Mailer tailored, personalized services to satisfy family needs and preferences.

It also determines the community's preference for traditional and cremation services and makes known the availability of payment plans.  The survey offers a Living Will to those who View PDFcomplete and return it. Offering a gift increases the number of returned surveys. Gifts offered are the responsibility of the survey purchaser.

Most States provide free copies of state approved Living Wills that can be downloaded from their websites. Copies can be made for distribution to survey responders.

This survey can be edited to provide different gifts or no gift. 

Survey Price: $594 per 1,000 / minimum purchase of 1,000. Call or email to obtain counts for the zip codes to mail.

This option is for buying a "LIST" of the names/addresses mailed to in your order. This list also includes phone numbers that are available.

This option is for printing the purchaser's own postage paid return envelopes with purchaser's return address in lieu of having responses sent to our response center.
